How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

Sometimes patients can register with the practice if they live outside of the practice area, but this can only be done in certain circumstances and with a Practice-Patient Agreement in place. If you wish to register with the practice but live outside of the catchment area please call the surgery on 01925 599855 and ask to speak to a member of the management team.

We are using a new online service called  https://digital.nhs.uk/services/register-with-a-gp-surgery-service/find-out-more-about-the-service-and-its-benefits that makes it easy to register with this GP surgery.

Just fill in this quick form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

You can register by completing a form available from reception. To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery.

You will also be required to complete an ethnicity form, alcohol form and a contact details form.

New patients may choose to register for SystmOnline at the same time (please see “Online Services“) and/or SMS text messaging service

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

We recommend that new patients undertake a health check with a practice nurse.
