🏠 Housing & Benefit Letters
At Holes Lane Medical Centre, we are committed to supporting our patients. However, any requests for letters to benefit agencies, the council, or other third parties must follow certain procedures:
General Rules
Requests must come from the third party and include your formal consent.
We will only provide factual information; we cannot give opinions.
If the request is unclear, we may refuse to complete it or offer a copy of your medical records instead, with your consent.
Requests are subject to a fee and can take up to 28 days to process.
🏡 Housing Applications
Important: You do not need a doctor’s letter to apply for a council home for health reasons.
The council may contact your GP directly if further information is needed.
Most housing applications are supported using a self-assessment form from the Housing Department. This form should not require GP input.
Only if additional medical details are requested will the Housing Department Medical Officer contact your doctor, and only with your written consent.
What Information May Be Shared
If requested by the Medical Officer, we may provide factual details such as:
Diagnosis
Severity of the illness
Current medication
Tip: Always ensure the Housing Department or Housing Association submits requests on your behalf, not you directly.
