You can request access to your medical records via:
SystmOnline – view your records online
NHS App – view your records, medications, and appointments
Written request to the practice – complete a subject access request form
Opting In or Out – Care Data Programme
Coded GP data may be extracted via the GP Extraction Service if no objection is made
You have the right to opt in or opt out before data extraction
